Medical/action Plans and Contact Details on the Day - Check Boxes

Would be great if the Medical/Action Plans and Contact Details on the Day questions are a yes/no question and only bring up a text box if they answer yes. This would save many emails coming through which are totally irrelevant.
Or make the Consent Form a template that the school can modify/design themselves so we can utilise these functions as per the Questions database in the Events module

Agreed, being able to modify the consent form would be wonderful (like being able to remove the credit card details section from paper consent form!)

Yes, families add a phone number but after checking personal details, turns out to be the standard contact number anyway.
